If you are living in Nova Scotia or another Canadian province, please call our office to request a Retirement Package. NOTE: The Application form cannot be submitted until you have received a personalized calculation sheet of your options. Contact the Nova Scotia Pension Agency for this.
If you live outside Canada, the application forms are available for download below.

Retirement Application Forms
Important - Please make note of the following points:
- Form #1 cannot be submitted until you have received a personalized calculation sheet of your options. Contact the Nova Scotia Pension Agency for this.
- Forms #1 through #5 must be completed in full and submitted to the Nova Scotia Pension Agency (incomplete forms may delay the processing of your application).
- Form # 6 must be completed - NOTE: Please return to the organization noted on the form.
- Form #7 is optional. NOTE: Please return to the organization noted on the form.
To apply for your pension you must complete forms #1 through #5 and submit to the Nova Scotia Pension Agency:

The Application for a Service Pension has complete instructions for returning all of the forms. Please read it carefully as there are three different addresses to which specific forms are to be sent. If you are having trouble downloading the forms, contact the Nova Scotia Pension Agency and we shall be pleased to mail them to you.
